This is the placeto find the place.

Skilled nursing facilities, ranked by inspection data.

If someone handed you a list of skilled nursing facilities this week, it came from a hospital discharge planner and the bed will not be held past Friday. Nobody taught you how to read it. Every place on it has a reassuring name and a photograph of the same sunroom.

You are choosing where a parent recovers from a hip, a stroke or a surgery — from a hospital corridor, while a case manager waits on you and your parent asks when they are going home.

A skilled nursing facility is a Medicare-certified nursing home: the same buildings as the nursing homes, asked a different question. For a short rehab stay, three things decide how it goes — the nurse hours a resident actually receives, whether therapy happens daily or quietly gets skipped, and how often people leave in an ambulance instead of a car. All three are in the inspection record.

This is what the inspectors wrote down.

Every ranking here is built from federal CMS inspection results — deficiency citations, staffing hours, penalties and quality measures. The same findings a surveyor filed. Not brochures, not tours, not what a facility says about itself.

You have days, not weeks. That is not the disadvantage it feels like.

The families who get this right are not the ones with the most time. They are the ones who look at three numbers instead of ten brochures. You already know which parent you are protecting and what going home is supposed to look like. Bring that. We will bring the record.

Common questions

What is a skilled nursing facility?
A skilled nursing facility is a nursing home certified to provide Medicare-covered skilled care — nursing and rehabilitation therapy after a hospital stay. The building is usually the same one; the term describes the level of care being billed.
Is a skilled nursing facility the same as a nursing home?
Largely yes. Most Medicare-certified nursing homes provide skilled nursing care for short rehab stays and long-term care for residents who live there. That is why 17,905 facilities appear under both terms on Palmelle.
How long does Medicare cover a skilled nursing stay?
Medicare Part A can cover a skilled nursing stay after a qualifying hospital admission, in full for an initial period and then with a daily coinsurance amount, up to a benefit-period cap. The amounts change annually — confirm the current figures with Medicare directly before relying on them.
We have to choose in two days. What should we look at first?
Nurse staffing hours per resident, the most recent inspection's deficiency count and severity, and the rehospitalization rate. Those three separate good short-stay rehab from bad faster than anything on a tour.
